Need help with PowerColor RX570 - Low Hashrate w/ mod + oc

Hey everyone, looking for some ideas. I've been fighting these cards for the past 3 days with no luck, and figured it's time to ask for help.

I have the PowerColor RX570 Red Dragon 4GB w/ Hynix. I've done everything I can think of and can't get my hashrate above 24.

Some notes:
0:xxxx in my notes is doing both (1:xxxx & 2:xxxx).
"tool" is using the doreymills.ca tool, "mem" is just changing the 1750 mem to 2000.
Dual mining with sia using Claymore
I'm on Windows 10 (current updates).
4GB RAM, 16GB virtual memory.
No risers, directly into x16 slots
AMD 1045T CPU

I've tried 3 different cards and am getting the same results on all of them. I even threw it on SimpleMining for a bit, same results.

If anyone can provide any insight on what might be going wrong I'd be grateful. I feel like I'm simply overlooking something, but I'm not sure.
